Mid Term Applications

If you would like to apply for a mid-term transfer for your son/daughter, you need to
complete an In-Year Common Application Form [IN-CAF]. Further information can be
found on the Wolverhampton City Council website:
http://www.wolverhampton.gov.uk/article/2547/Changing-schools
If you are applying for a Foundation Place (for families who are faithful and regular
worshippers of a church which is a member of Churches Together in Britain and
Ireland or Churches together in England) you must also complete a Supporting
Evidence Form (SEF) which can be found below together with our Admissions Policy
and Help & Guidance notes for Church leaders. Please read these carefully to assist you & your Church leader in completing the form.

You must complete details of your faith commitment, along with support from your
church leader. If the church leader is unavailable, then another member of the faith
community in authority can complete it on their behalf. Attach all relevant evidence to
support your application. Use a separate sheet if necessary.

Please be aware that siblings are not guaranteed a place so you must fill in ALL
sections of the form to show us about your faith commitment. We can only take into
account the information presented on the SEF, so please tell us all relevant
information about your child, including any medical or pastoral needs.
The completed Supporting Evidence Form must be returned to the Admissions Team
at St Peter’s Collegiate Academy.

If you do not complete a Supporting Evidence Form your application will be
considered under an Open Place.

If we are over subscribed families will be added to the waiting list as detailed in the
Admissions Policy listed above.
Appeals

Families who have made an application for their child to be admitted to the Academy
but have been unsuccessful, have the right to Appeal. If you wish to appeal please
call the St Peter’s Admissions Team on 01902 558600 option 5 or
email admissions@stpetersacademy.org.uk in the first instance.

Details of your appeal should then be sent to School Admissions at City of
Wolverhampton Council and full details can be found here: Independent Admission
Appeal Form | City Of Wolverhampton Council
Once received by the Clerk to the Appeals panel at the Civic Centre, a hearing for
your case to be heard by an Independent Appeals Panel will be arranged. You do
not have to be present at the Appeal as it will still be heard in your absence. Also
present at the appeal will be a representative from the Academy.

Once all hearings have taken place, a decision will be made and you will be informed
in writing of the outcome.

If you have any questions, please contact the Academy Admissions
Department on 01902 558600 (option 5).
